Our patients can access Colorado family medicine at five convenient Middle Park Health locations throughout the region- Winter Park, Granby, Grand Lake, Kremmling and Walden

  • Middle Park Health – Granby Campus offers family practice hours five days a week along with walk-in care on Saturdays. You’ll find the Granby Campus on U.S. 40 in Granby just south of Kaibab Park and around the corner from the transit station on Railroad Avenue.
  • Middle Park Health – Kremmling Campus has weekday family practice hours. The Kremmling Campus is located just south of U.S. 40 on 4th Street.
  • Middle Park Health – Grand Lake Clinic offers family practice appointments Wednesday through Friday. This clinic is just one block east of the Grand Lake Post Office in the Community Center Building.
  • North Park Medical Center serves those who live in Walden and surrounding communities. Residents can find us just a block away from Vic & Sigrid Hanson Memorial Park.
  • Middle Park Health – Winter Park Walk-in and Family Practice Clinic offers both walk-in and scheduled family practice appointments. This facility is conveniently located by the Cooper Creek Square in downtown Winter Park.

Our family practice locations have experienced physicians and advanced practice providers who provide primary care to our patients throughout the region. The family medicine physicians at this location offer:

  • Well visits: The American Academy of Pediatrics recommends annual check-ups from ages two to 21 and more frequent visits for newborns, infants, and toddlers. Adults should also visit their primary care physician every year or two.
  • Preventive care: Our family medicine physicians provide recommended vaccines and health screenings depending on your age and health risk factors. For example, adults should be screened for high blood pressure, high cholesterol, and diabetes. You’ll find these services at MPH.
